Every deviation record starts with a paragraph that says what happened. In most of the records we review, that paragraph is where the investigation goes wrong, and it goes wrong before anyone has investigated anything. The problem statement either contains a cause, or it contains a conclusion, or it is so vague that the investigator is free to find whatever they expected to find.
